All employees, regardless of
their insurance carrier, can now visit an on-site Cor Medical clinic to receive
their biometric screenings. Download the Health Provider Screening Form on the
Quest Diagnostics site (access through Rally)
and bring it to your appointment at the clinic. Remember: you can earn $150
when you complete both the biometric screening and the rally health survey.

Cor Medical, an independent medical company, operates worksite clinics at Lumen in Monroe, Louisiana. The clinic serves the medical needs of employees, spouses, domestic partners, and dependents (children between the ages of 18 and 26). All must be enrolled in a Lumen medical plan to qualify.
Clinic providers—through in-person and virtual visits—treat a variety of medical needs, including, but not limited to:
